* fix hls: require fresh origin window for new playback on retained sessions, count known-length revision bodies as completed - New playback leases joining a retained shared session no longer start on a stale committed window. If the origin window was last confirmed more than two target durations ago, admission requires a fresh commit (StaleEntryWindow) using the strict pinned host and no recovery burst. This prevents the forward jump after fast-start entry. - RevisionResponseStream tracks bytes sent. A known-length body that is dropped after its final byte is now recorded as completed and marks segment completion instead of logging outcome=dropped. - Add an end-to-end re-entry regression test covering progressive, first_ready and conservative, plus unit tests for the commit requirement and revision completion.
