- Fixed SQL to create temp table for lookup of last backup's file
entries
- Removed temp table by writing directly to internal lookup struct.
- Direct lookup of single file entries from DB

Kenneth Skovhede a350d942e6 Fixed a case where a specially crafted broken index file could prevent the database from being recreated correctly. 
This fix also optimizes recreates where data is missing in index files and block files are downloaded.
