* Save: ask before overwriting a file another app changed (#6596)

File › Save now notes the size and modification time of a document's
own file when it is opened or saved, and checks them again before
writing back. If another app changed the file in between, a prompt asks
whether to replace it (Cancel keeps both; Save As saves elsewhere)
instead of silently overwriting the other app's changes.
